Log-In
Your username is the email you used when you registered. If you forgot your password, click on the forgot password button to reset it. If you need help accessing the Patient Dashboard (Portal), please email portalhelp@eastersealsdfvr.org. Please include the client’s name and date of birth, and your relationship to the client.
If you (or the client’s other parent/legal guardian) do not have a log-in, please email the portalhelp@eastersealsdfvr.org email with your name and relationship to the client so that we may send a Welcome Letter.
Updating Your Information
To update your address, phone number, emergency contacts, or insurance:
- Click the Profile button on the left menu.
- Review and edit your Personal, Contact, and Insurance information.
- Note: If you have multiple children, ensure you are in the correct account.
- Be sure to answer questions from the child’s perspective.
Accessing Records (Visit Summaries)
To view or print reports (Evaluation and Progress summaries):
- Click Records, then select Visit Summaries.
- Allow a moment for the list to load.
- Select the desired report to view or save as a PDF.
Uploading Documents
To securely share reports or pictures with your care team:
- Click Records → Upload Documents.
- Select Open Secure Message and Upload Documents.
- Type a brief message, click Browse to select your file, and choose the document category from the dropdown menu.
- Click Add Another or Send Message to finish.
Payments & Billing
- Click Make a Payment to view your statement summary.
- Select Pay Now to pay a balance or co-pay.
- Secure Storage: You can add and securely store a payment method here (required for co-pays).
- Payment Plans: You may propose a payment plan through this feature. Note: All plans must be approved by the Billing Department. Contact Yolanda DeLuna at ydeluna@eastersealsdfvr.org for assistance.
Attendance & Cancellation
- Requirement: Please maintain at least 85% attendance. Falling below this over three months or missing two consecutive weeks without notice may result in discharge.
- Running Late: Late arrivals will result in a shortened session. Frequent lateness will require a meeting to review your schedule.
- Fees (non-EI clients):
- No-Show: $125 (if you do not contact before appointment starts)
- Same-Day Cancel: $75 (Waived for sudden illness or if you reschedule/switch to teletherapy).

With these procedures in place, we have had minimal disruptions in our services, whether at a Center or via tele-therapy. We are happy to change an appointment to tele-therapy when attending in the center is not possible.
