ESSENTIAL FUNCTIONS: 

1. Ethical Standards of Practice 
2. Delivers services consistent with established Easterseals DuPage & Fox Valley Mission, Values, and Philosophy. 
3. Maintains patient confidentiality and respects the rights and dignity of all individuals. 
4. Adhere to Easterseals DuPage & Fox Valley Code of Ethics.  Comply with all Federal, State and local laws and regulations.   
5. Complies with current ASHA Code of ethics, state licensure laws, and all prevailing Federal, State and local laws regulations. 
6. Evaluations    
7. Screens, consults and/or evaluates patients upon request/referral. 
8. Performs evaluations/screenings within Center’s formats of   

• IDE 
• Single discipline 
• Reviews relevant reports to facilitate assessment. 
• Selects appropriate evaluation tools and implements them, as indicated.  

3. Makes appropriate recommendations from evaluation results. 
4. Writes evaluations in accordance with Center policy. 
5. Administration of Therapy 
6. Establishes a Treatment Plan from initial evaluation. 
7. Administers program effectively: 

• Utilizes appropriate treatment procedures and techniques. 
• Adapts treatment to patient’s needs and abilities. 
• Identifies resources and integrates information on resources into program planning and implementation. 

3. Continually evaluates, predicts program outcomes and updates Treatment Plan based upon client’s/family’s changing needs. 
4. Meets current productivity expectations as outlined by the department. 
5. Provides appropriate client and family education and training. 
6. When appropriate, acts in coordination with other therapies in planning therapy program, including presentation in MAB and IDS meetings, as necessary. 
7. Communicates with other internal and external team members, referring physician, and other relevant stakeholders as authorized by client family/legal guardian. 
8. Serves as client case manager, when assigned. 
9. Establishes the discharge/transition plan and refers client to other services/programs, as needed. 
10. Provides follow-up procedures: 

• Provides home program. 
• Increases or decreases therapy as indicated. 
• Discharges when appropriate or refers to appropriate alternative placement.                    

11. Sanitize toys and treatment space.

12. Clinics – therapists will participate, and coordinate responsibilities outlined by specific clinic requested. 
13. Attends and participates in school staffings, clinics, etc., as requested. 
14. Assists in recommending and ordering assistive communication equipment, when necessary. 
15. Therapist will participate in fulfilling Community Based programs/contracts as requested. 
16. Job duties and expectations specific to contracts will vary in accordance with the terms of the agreement between this Center and the agency contracting our services. 
17. For the Center developed Community Based program (s), job responsibilities and expectations will be reviewed with the Community Based Therapy Manager. 

Documentation 

Maintains efficient records: 

1. Writes patients progress reports and evaluations in accordance with Center’s policy. 
2. Serves as case manager when assigned, acting to help in the coordination of all documentation needs. 
3.  Assist with finance department’s needs relating to documentation. 
4.  Prepares discharge upon termination of services. 
5.   Acts to assure therapy records are complete upon clients’ discharge.  
6. Department Program Development 
7. Participates in the planning for and training of SLP students. 
8. Attends staff meetings, interdepartmental meetings, in-services, clinics, and Medical Advisory Board, as necessary. 
9. Helps to maintain department supplies, equipment, and toys. 
10. Participates in the development of and the review of department policies and procedures relating to treatment, documentation, and program development. 
11. Maintains and develops professional skills through the pursuit of continuing education and membership in professional organizations. 
12. Promotes nurturing/mentoring of fellow staff members through sharing of information and the commitment to a team approach. 
13. Participates in department committees/ team department responsibilities as requested.  

QUALIFICATIONS:  

1. Must be licensed or license eligible as defined by ASHA and Illinois Board of Professional Regulation. 
2. Must have a Master’s degree or a reciprocal ASHA approved certification.   
3. New graduates must make application for State licensure within one month of completion of requirements for ASHA Clinical Competency Certification in Speech-Language Pathology. 
4. New graduates must have registered for the ASHA exam within 3 months of hire and completed within 9 months of hire. 
5. Must maintain EI certification or eligibility. 
6. Must maintain current CPR certification. 
7. Must have additional professional development, experience, or training to identify as an Assistive Technology or Feeding Specialist.  

REQUIREMENTS: 

Unless exempted by the Americans with Disabilities Act, all persons hired for this position are required to perform the following essential functions:   

1. Lift up to 50 lbs. on an as-needed basis 
2. Perform the basic life operational functions of talking, hearing, vision  
3. Perform repetitive motions (i.e., computer keying and/or dialing phone) 

This position requires exposure to the following environmental factors: 

1. Daily contact with domestic and industrial cleaners.  

2. Exposure to variable noise levels.  

3. Occasional contact with noxious smells.  

4. Frequent exposure to communicable illnesses
