Easterseals offers parents and caregivers free access to the confidential online Ages & Stages Questionnaires®, Third Edition™ (ASQ-3) and the social emotional tool ASQ SE-2. The ASQ-3 is a set of questionnaires that helps you track your child’s development from one month through age 5. Trusted by pediatricians (Lipkin, et al., 2020) and used for more than 20 years, this screening provides a quick look at how children are doing in important areas, such as communication, physical ability, social skills, and problem-solving skills. ASQ-3 can help identify your child’s strengths as well as any areas where your child might need support.  

Here’s How ASQ Works  

1. You will answer each question “yes,” “sometimes,” or “not yet,” based on what your child is able to do now. Your answers help show your child’s strengths and areas where they may need practice.
2. To answer each question, you can try fun and simple activities with your child. These activities encourage your child to play, move around, and practice day-to-day skills
3. After you complete the questionnaire, a professional from Easterseals will share the results with you.  

You may try the next screening age level as your child grows and learns new skills. If your child has trouble with some skills, Easterseals will help you with next steps. Finding delays or problems as early as possible supports young children’s healthy development.   

• The ASQ-3 (developmental screening tool) may be taken every 2-, 4- or 6- month period.
• The ASQ SE-2 (social-emotional screening tool) may be taken every 6 months until your child is 3 years old, and then once a year at 4 years and 5 years.
